Position Overview

MetroPlus Health’s Partnership in Care (PIC) program provides a team-based approach to caring for members who are living with HIV, homelessness, or transgender. PIC provides comprehensive care management services through a team of care managers. The intern would oversee a project related to improve quality measure performance such as diabetes outcomes or preventative care screenings. The primary focus would be to plan and implement a quality improvement project with the goal of improving performance through direct member outreach and collaboration with providers/facilities. The internship position will report to the Community Outreach, Quality, and Compliance Director and work closely with the Care Management team and Medical Director of Partnership in Care.  

Duties & Responsibilities

  • Identify root causes and barriers to quality measures through research, brainstorming with care managers/leadership, and analyzing available data
  • Plan/implement QI project with the goal of improving rates of quality measures
  • Coordinate with the care managers assigned to member to conduct targeted telephonic outreach to PIC members with gaps in care
  • Assist members with scheduling of appointments for services
  • Address member’s barriers to completing services/appointments
  • Develop and implement strategies/tools to help members close gaps in care
  • Collaborate with providers/facilities to close gaps in care
  • Analyze impact of project on rates of quality measures
  • Participate in other special outreach and quality improvement projects as assigned

Work Shifts

9:00 AM- 5:00 PM

Minimum Qualifications

  • Proficient with Excel; Strong verbal/written communication skills; Experience with community/patient outreach
  • Interest or experience with the HIV population, community outreach or working with underserved populations, and quality improvement is preferred
